Individual Consumers
If you are a member of the Settlement Class as a consumer, you may file a claim for a share of the Settlement Fund. You must complete a Claim Form and mail it to the Settlement Administrator, postmarked no later than April 17, 2025, or you can submit your claim online no later than April 17, 2025.

Third-Party Payors
Third-party payor (“TPP”) Settlement Class Members are entities that purchased, paid, and/or provided reimbursement for prescriptions by their members, employees, insureds, participants, or beneficiaries. They include entities such as health insurance companies and self-insured health and welfare plans.
If you are a member of the Settlement Class as a TPP, you may file a claim for a share of the Settlement Fund. You must complete a Claim Form and mail it to the Settlement Administrator, postmarked no later than April 17, 2025, or you can submit your claim online no later than April 17, 2025.
